Questionnaire templates

Questionnaire templates let a coach prepare reusable intake and follow-up forms. A template can be selected when inviting a family or used later to add another questionnaire in Session prep.

Who this guide is for

This guide is for subscribed coaches managing questionnaires in the coach portal.

What you can do

  • Create several questionnaire templates.
  • Organize questions into sections.
  • Use text, number, choice and yes-or-no questions.
  • Mark questions as required and add help text.
  • Duplicate a template.
  • Choose the default template.
  • Remove a template from future use.
  • Assign another questionnaire to a current family.

Create a template

  1. Open the questionnaire manager in the coach portal.
  2. Choose New.
  3. Give the template an internal name that helps you recognize it.
  4. Add the family-facing title and optional introduction.
  5. Edit the starter question or add more questions.
  6. Use Save when the template is ready.

The internal name appears in coach tools. The family-facing title and description are what the family sees.

Build the questions

For each question:

  1. choose or enter its section;
  2. enter the question text;
  3. choose the response type;
  4. decide whether it is required; and
  5. add help text if the family may need an explanation.

Available response types are long text, short text, single choice, multiple choice, yes/no and number.

Choice questions need at least one option. Turn on Ask for details for an option when the family should be able to add text. An option called Other starts with this setting enabled.

Use the up and down controls to arrange questions. At least one question must remain in a template.

Save and manage templates

Changes remain unsaved until you use Save. An Unsaved badge marks local edits. You can switch between templates while staying on the manager, but leaving or reloading the page can lose unsaved work.

Useful actions include:

  • Duplicate — creates a separate copy. It includes the content currently visible in the editor, including unsaved edits.
  • Set as default — makes the template the starting choice for future invitations.
  • Remove — removes the template from future selection without deleting questionnaires already sent to families.

Only one template is the default. You cannot remove the final active template, and removed templates cannot currently be restored from the manager.

Avoid editing the same template in two browser tabs. The last saved tab can replace changes saved from the other one.

Choose a template for a family

When inviting a family, choose an active template, keep the default, or choose Send later / no questionnaire yet.

Once an invitation is created, later template edits do not change the questions selected for that invitation.

To add a questionnaire later:

  1. Open the family's Session prep area.
  2. Choose Add questionnaire.
  3. Select an active template.
  4. Confirm the addition.

Each addition creates a separate questionnaire. It does not replace earlier questionnaires or answers.

Things to know

  • Editing a template never changes questionnaires already sent to families.
  • Changing the default affects future choices only.
  • Removing a used template keeps all existing family questions and answers.
  • Families see only questionnaires assigned to them, not your full template list.

If something goes wrong

  • If save fails, your local edits remain available. Correct the highlighted problem and try again.
  • If a choice question will not save, make sure it has at least one non-empty option.
  • If remove is unavailable, create another active template first.
  • If a family has no questionnaire, check whether Send later / no questionnaire yet was chosen, then add one from Session prep.
